Steps to Register

Step 1

Complete the Voluntary Disclosure Form. If you are not sure what accommodations may be helpful, you may discuss this with the Accessibility Staff in person. If you need help completing this form, please contact the office. 

Step 2

Review the Guidelines for Documentation. Appropriate documentation is a part of determining your eligibility for certain accommodations. It can be helpful to share these guidelines with your clinician(s) to aid them in understanding office expectations for documentation.

Step 3

Review the Policies and Procedures webpage, then complete and return the Receipt of Policies Form.

Step 4

Submit forms and supporting documents via:

Step 5

Email, call (828.232.5050), or come in to schedule an appointment to discuss next steps. The OAA will send all official communications to the student’s university email address and/or in writing to the student’s permanent address listed with the university. 

The University may recommend seeking additional support from agencies such as the Division of Vocational Rehabilitation, Division of Services for the Blind, Division of Services for the Deaf and Hard of Hearing, etc.
